It is an attempt to provide you information to be able to identify those in your community who will be most receptive to PG.

You do not have to answer anything you do not want or provide details which will identify you.

Survey Monkey only allows for 10 question surveys unless you are a paying member. Therefore it may be prudent to perform multiple surveys depending on the response rate.

If you would like to create a survey to further identify the people we should be looking for, please do, but please also forward that information to me as well. I would use anon email in signing up to create the survey and you can get an address here
